Notify your direct supervisor, department head
and/or Human Resources Office immediately.

The Traveler’s Nurse will complete the first report of injury and direct care.
You may be issued a prescription card by the Traveler’s Nurse.
Follow the medical care directed by the Traveler’s Nurse or go to one of the following locations:

A First Report of Injury must be completed within 24 hours from the time of the incident, and in order for 探花社区 to consider if the injury/accident will qualify for workers compensation insurance coverage. Please log into , and under the Employee tab go to the Benefits and Payroll Forms link on the left. Under Benefits Forms you will find a link to Work Injury – Workers Compensation.
